New Patient Appointments:
Please arrive 10-15 minutes early to your appointment for registration and to fill out all of our new patient documents and allow us to get copies of your insurance information. When you come for your appointments, please remember to bring the following
- Medicare card
- Insurance information
- Driverís License or Picture ID
- Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.
- If you have had surgery elsewhere, please bring a copy of your operation report.
Very often during the course of your evaluation or treatment for an orthopaedic condition we will require x-rays to determine the cause of a problem or to evaluate your progress.
Patients may also visit our web site www.markrickettsmd.com to download Patient Forms before an appointment or you may set up an account on our Patient Portal. The Patient Portal will allow you to manage your health information and documentation.
Now that we have settled into our new location, we want to put our focus into continuing to improving our patients' experience. It would be greatly appreciated by myself and our office staff to get your feedback from your experience with us. Positive, negative, any suggestions - anything and everything - we would like to know.
And please be as candid as possible. Your feedback is kept completely confidential - in fact, it does not come to me. All comments are sent directly to an outside firm that assists my office in creating as positive an experience as possible for our patients, who then compiles the information so that we can evaluate our practices.
Your input and direction is an invaluable piece to helping us improve as health care providers.
